Fill out everything in the top portion, from "Name" through "Tel." as well as the first 5 sections of "Ranking Information."  Skip "registration number."

Fill out everything in the top portion, from "Name" through "Tel." as well as everything in "Ranking Information." If you have qualifications, list them here.

Sample Qualifications Exam

This sample is for a person taking the Instructor C exam and the Judge D exam.

Fill out every section from "Name" through "Rank of Dan."  If you have passed a prior qualifications exam, list your level and qualifications registration number in the "Holding license (currently)" section.  Check off which exams you are taking in the "Testing License" section.  If required for your exam, list the kata you are performing.
